Questionnaires can be generated based on both sending and receiving organizations within blockit. The benefits of questionnaires allow for accurate patient routing and data + analytics on the questionnaire answers. Questionnaires can be made optional or required.
Types of Questionnaires:
Pre-Screening (primarily for consumer scheduling) allows a questionnaire to be displayed prior to the provider search results page. This is used to alter or filter initial search results based on question response.
- Figure 1 below is an example of a pre-screening questionnaire. This pre-screening questionnaire will route the scheduler or patient to a filtered provider listing based on how the questions were answered.
- Figure 2 below is an example of help text that can be configured for the pre-screening questionnaire to provide any additional details needed for a question.
In-Line (consumer and referral scheduling) allows a questionnaire to be displayed on the scheduling form prior to booking. This option can be positioned before the patient demographic information fields or after. This is used to capture additional information and allow for controlled hard-stops/notices if the patient criteria does not meet the receiving provider's requirements. The receiving practice's EMR will determine if questionnaire responses are able to be carried over into their EMR.
- Figure 3 is an example of an inline questionnaire with varietal options. The in-line questionnaire allows for additional data to go to the receiving practice or stop the patient scheduling process if the answers do not align with the receiving practice's requirements.
The blockit app offers the following question formats:
- Text (single line for text with minimum of 1-2,000 characters)
- Text Area (free text box)
- Boolean (yes/no with a hard-stop/disqualifier preventing scheduling if Yes is selected)
- BooleanInverse (yes/no with a hard-stop/disqualifier preventing scheduling if No is selected)
- Radio (yes/no without hard-stop, a bubble button, horizontally positioned)
- RadioVertical (yes/no without hard-stop, a bubble button, vertically positioned)
- Select (drop down menu selection)
- Checkbox (checkbox selection)
- Date (date selection for month, day, year)
- Time (time selection for hour, minute, a.m./p.m.)
- html (static text)
- Upload (allows for document upload of image, word, and pdf files with a size limit of 20mb)
- InsurancePlans (dropdown select field with dynamically populated list of insurance plans)
Things to Note:
- Questionnaires and subsequent features can only be activate and configured by submitting a request to the blockit support team.
- The client can choose how questions are ordered, verbiage, format, and if the questions are required or not required.
- Branching logic is currently applicable to consumer scheduling, not referral.
